At Pathways ECI, we ensure that the services we provide to your child and family are relevant and effective by having an Individual Family Service Plan (IFSP).This is a planning process that helps everyone to work in a coordinated way to meet your needs.

You decide the goals that reflect your family’s concerns and priorities. We encourage and support you to coordinate the planning, as this is an important skill for the future. A meeting is often held to bring everyone together for planning. You can choose where to hold this meeting and you also decide who to invite.

Families usually have this meeting once or twice a year to keep everyone focused on what will make a difference. Working in partnership with you is the key to a good plan. Please contact a staff member if you would like to have an IFSP meeting.

What is an Individual Family Service Plan (IFSP)?

A written plan which outlines specific goals for your child’s development which are based on your family’s priorities.

Your goals should be measureable and achievable.

The plan is usually developed in a meeting where the different professionals who are currently working with your family can share strategies, which will help you acheive your goals.

You should review at least twice per year helps to monitor progress and make changes if necessary.

A copy of your IFSP is also required for your Family Assistance Fund application and it must support your equipment or resource request. The Family Assistance Fund, funded by the NSW Government, provides families with $2000 per year to strengthen their well-being and support their capacity to care for their child. Our staff can assist you to find out if you are eligible and help you to apply for this funding.
